WORKING TOGETHER TOWARD A HEALTHY FUTUREAmerica's senior population is growing at an unprecedented rate. Colorado ranks among the top five states in senior-population growth. The Colorado Demography Office projects that the population age 65 and over of El Paso county will increase from 61,788 in 2010 to 172,394 in 2040. This 179% increase is more than triple the increase in total population! To accommodate this population explosion, we must expand and evolve health care for seniors. As costs increase and the economy struggles, many seniors are now less able to afford it. Others face barriers of mobility and provider access. Even when these barriers are overcome, most senior care targets problems in isolation - rarely addressing all relevant health factors in an integrated way. Fortunately, the region is home to a remarkable partnership, between the University of Colorado Colorado Springs and Peak Vista Community Health Centers, that has been addressing these challenges for years. Through a generous donation from Margaret L Lane and the John E. & Margaret L. Lane Foundation, Peak Vista plans to relocate and expand an existing senior health center on to UCCS Campus. This new facility will be called the Lane Family Senior Health Center and will be located within the Lane Center for Academic Health Sciences. When completed in 2014, the Lane Family Senior Health Center will feature innovative inter-professional healthcare models and deliver them in a convenient location - giving patients "one-stop shopping" for treatment, prevention, and support. The Peak Vista/UCCS partnership will also spur new research and provide integrated care – in which physical, mental, and other health factors are considered jointly as part of a single clinical experience. The result? Better health outcomes that achieve patient goals. |

Should funding needs change, the Board of Directors of Peak Vista Community Health Centers Foundation is authorized by these policies to use the funds for purposes similar to the original purpose that in their judgment will help advance the aims of promoting access to healthcare. |
