Patient enrollment services are offered at seven different locations with over 40 staff members dedicated to enrolling patients into Peak Vista as well as assisting with Medicaid, CHP+ and Colorado Indigent Care applications.

The time it takes to complete an application varies with each individual situation and family size but average time is approximately 1 hour. Patients must be enrolled at Peak Vista to receive care.

• If you have active, Medicaid, CHP+ or accepted private insurance, you may enroll over the phone by calling (719) 632-5700.
• Some documentation is required to enroll. Please read our Enrollment Information page to learn what to bring with when you enroll at Peak Vista.
