Warfarin Clinic

The purpose of the Warfarin Clinic is to assist Peak Vista Community Health Centers providers in managing patients taking the anti-coagulant drug warfarin (Coumadin). Warfarin is a difficult drug to dose due to considerable variability with interpatient and intrapatient response, lack of dose/response relationship, and significant dose/response lag time. Pharmacists in the Warfarin Clinic have received special training and work under a protocol developed to assist providers in obtaining optimal anti-coagulation therapy and to avoid adverse drug reactions. Patients must be referred to the Warfarin Clinic by their Peak Vista provider.
