Peak Vista Community Health Centers and Mountain View Medical Group have joined forces as an Accountable Care Organization (ACO) to participate in the Medicare Shared Savings Program (MSSP). This new partnership, known as Clinical Partners of Colorado Springs (CPCS), is made up of physicians and other health care providers who work together with Medicare to deliver high-quality service and care at the right time, in the right setting. Under the ACO model, physician-led Care Teams provide coordinated care for Medicare Fee-for-Service beneficiaries that are enrolled in traditional Medicare.
The primary goal of Clinical Partners of Colorado Springs (CPCS) is to improve and maintain population health by providing a high-quality continuum of care. As participants in the MSSP, CPCS facilities and providers will be measured on 33 separate quality indicators to ensure appropriate, timely and well-coordinated care
CHP+ is low-cost health insurance for Colorado’s uninsured children and pregnant women. CHP+ is public health insurance for children and pregnant women who earn too much to qualify for Medicaid, but cannot afford private health insurance.
