What is the Chronic Care Managment (CCM) program?
  • Chronic Care Management (also referred to as CCM or Comprehensive Care Management) is a program at Peak Vista offered through Medicare.

  • You do not need to come to our office for these services. Most of these services will be provided over the phone, from the comfort of your home, between doctor visits.

How will the CCM program benefit me?
  • Once enrolled, you will be assigned a Care Coordinator who will call you to help with your health conditions.

  • Your Care Coordinator will help you get medication refills, schedule appointments, and coordinate other aspects of your care like transportation, community resources, and medical equipment.

  • This program is for Medicare or Medicare Advantage patients living with diabetes, high blood pressure, heart disease, lung disease, arthritis, and other long-term health conditions.

What do I need to know before signing up?
  • Medicare will cover a majority of the cost and any additional coverage will be billed to your secondary. Your usual and customary cost share will apply.

  • You can stop these services at any time by letting us know.

  • Our enrollment specialist may call you to discuss this program.

Remote Patient Monitoring

What is Remote Patient Monitoring (RPM)?

Remote patient monitoring is a form of telehealth, or remote health care. Mobile devices, such as blood pressure and glucose monitors are used to collect patient data outside of traditional health care settings, such as your home. This data is electronically transmitted securely to your health care team allowing them to monitor vital signs relevant to your conditions.

How will the RPM Program benefit me?

As a participant in our RPM program, you will receive a blood pressure / glucose monitoring device. The device will allow your doctor to monitor your health status and track your readings without leaving your home. If your readings are out of the normal range, your care team will be alerted and will get you the care you need.

What do I need to know before signing up? 

Medicare will cover a majority of the cost and any additional coverage will be billed to your secondary. Your usual and customary cost share will apply. Our staff will assist you in setting up and using the device. These devices are designed to be easy to use and only require cellular reception.
