Peak Vista Community Health Centers and Mountain View Medical Group have joined forces as an Accountable Care Organization (ACO) to participate in the Medicare Shared Savings Program (MSSP). This new partnership, known as Clinical Partners of Colorado Springs (CPCS), is comprised of physicians and other health care providers who work together with Medicare to deliver high-quality service and care at the right time in the right setting. Under the ACO model, physician-led Care Teams provide coordinated care for Medicare Fee-for-Service beneficiaries that are enrolled in traditional Medicare.
The primary goal of Clinical Partners of Colorado Springs (CPCS) is to improve and maintain population health by providing a high-quality continuum of care. As participants in the MSSP, CPCS facilities and providers will be measured on 33 separate quality indicators to ensure appropriate, timely and well-coordinated care.
For general or additional information about Accountable Care Organizations, please visit Medicare or call 1-800 MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.
CPCS has an established governance body, and will publicly report its earnings and outcomes on this webpage. The following is organizational and contact information for CPCS:

Clinical Partners of Colorado Springs is a partnership between a Federally Qualified Healthcare Center (Peak Vista Community Health Centers) and a group medical practice (Mountain View Medical Group).
The distribution of shared savings payments by Clinical Partners of Colorado Springs will have a specific hierarchy. First, we will reimburse ACO start-up, operational and management expenses. It is estimated that reimbursement of ACO start-up, operations, and management expenses will make up approximately 40% of the shared savings distributions in the first performance year.
The remaining estimated 60% of shared savings will be retained by the ACO for the purposes of distributions to ACO Participants, and for reinvestment in improved infrastructure and care processes.
