FNP Fellowship (Apply Now)

The following items are required to complete the application process for the Peak Vista Community Health Centers Advanced Practice Registered Nurse (APRN) Primary Care Fellowship Program. All exceptions are at the discretion of the Program Director.

• Completed PVCHC APRN Primary Care Fellowship Application

• Must have at least 2 years of RN work experience

• Graduate of a Master or Doctoral level Family APRN program (with less than 750 hours of supervised prescriptive practice, as defined by Colorado DORA) or transitioning FNP who demonstrate an intent to practice as a primary care provider

• Transitioning Nurse Practitioners will be considered on an individual basis

• Certification in Primary Care specialty

• Eligible for Active & Unrestricted Colorado RN & NP license

  • Must be complete by the start of program.

• Ability to commit to 12-month, full-time program

• Successful completion of pre-employment screen, which includes, but is not limited to a Criminal Background check

• Other requirements as determined by Peak Vista Community Health Centers

• Three (3) letters of recommendation

  • One of which must be from a clinical preceptor

• Submit a 500-word essay answering the following:

  • What are your professional values as a nurse practitioner?

  • Tell us about your experience working with people facing access to barriers and your commitment to working with the underserved population.

  • There are several important roles within a team dynamic. Describe a successful team you were part of and how you contributed.

  • Describe a time in a professional setting in which you had a conflict with a supervisor or colleague. Give an example of how you managed the situation.

  • Discuss your expectations and goals of the Fellowship.
