The Peak Vista Community Health Centers Patient Advisory Group (PAG) serves as a formal mechanism for integrating the voices of PVCHC, patients, and families into our programs and services. The work of the PAG is based on the values of partnership, collaboration, engagement, respect, transparency, and a foundation of ensuring that healthcare services are patient and family centered.
The PAG exists as a means to acquire input from a diverse patient demographic, providing PVCHC insight into the experiences, concerns, and priorities of the patients they serve. Through the group, Peak Vista seeks to amplify and understand the patient and family/guardian’s perspective when designing or evaluating processes, services, environment, equipment, or patient communication and enhancing the patient experience.
PVCHC Patients, families/guardians, and staff actively engaged and partnered to understand, improve, and enhance the patient and family experience of our programs and services
Through advising, we provide the voice of our communities and advocate for a patient and family centered, quality health system that is accessible and sustainable.
A PAG Member may be removed from the PAG by agreement of the Chair, Co-Chair and BOD Member if the PAG Member misses three meetings.
