• Request prescription/medication refills, medical records, and appointments

• Access the patient portal by clicking the button below or by going to peakvista.org/portal.
• Click “Let’s Get Started.” Enter your email address and create a password when prompted.
• When asked for your pin, select “No.”
• Complete the process by confirming your email address for future logins.
