Our clinical pharmacists are trained to help with insulin and medication changes, blood sugar checks, continuous glucose monitors, and patient-specific diabetes education. They work under a protocol that allows them to adjust your treatment safely and quickly, in partnership with your provider.

We can help with:

  • Insulin support and safe dose changes
  • Blood sugar checks and how to understand your numbers
  • Setting up and using continuous glucose monitors (CGMs)
  • Reviewing all your diabetes medicines
  • Making sure your medicines work well together
  • Answering questions about refills, timing, or side effects

  1. Do I need a referral?
    Yes. Your Peak Vista provider will send a referral so our team knows how to best support your care.

  2. Who can use this service?
    Patients who receive Primary Medical Care(opens in a new tab) at Peak Vista and have diabetes and/or high cholesterol.
  1. Does this replace my primary care provider?
    Your primary care provider will continue to manage your other health concerns while our team focuses on your diabetes and heart risk. Our team keeps your provider updated, and once you’ve met your treatment goals, your primary care provider will resume managing your diabetes and/or heart risk.

  2. What does a clinical pharmacist do in my care?
    Our clinical pharmacists review your medicines, teach you how to use them, help with devices like glucose monitors, and adjust doses when needed.
  1. How often will I need to come in?
    It depends on your care plan. Some patients come every few weeks, others come in every few months, and some receive phone calls from our team more frequently.

  2. What should I bring to my appointment?
    Bring your glucose meter (if you use one), a list of your medicines, and any recent lab information.
  1. Can my medicines be adjusted during the visit?
    Yes. Our clinical pharmacists can make safe medication changes to best suit your needs and goals.

  2. Can I get help with insulin or continuous glucose monitors (CGMs)?
    Yes. We help with insulin training, CGM setup, and troubleshooting.
