Helping Outside Our Health Centers

At Peak Vista, our dedicated Community Health Workers (CHWs) play a vital role as frontline public health workers. They are committed to facilitating access to a wide range of health and social services that are crucial for individual well-being.

In addition to aiding with both medical and non-medical appointments, the CHWs are dedicated to creating access to essential provisions such as food, medications, and lifestyle support. Their approach is centered on meeting patients where they are, whether that be in their homes, a facility, or even on the streets.

CHWs have a significant and positive impact on our patients' health outcomes. They actively reach out to individuals who may not be aware of the services available, while also addressing the needs of existing patients and understanding the obstacles they face. 

Furthermore, our CHWs work in close collaboration with community partners to foster strong relationships and develop innovative ways to expand and fortify trust within the community. This multi-faceted approach ensures that individuals receive the support they need, while also striving to bridge any gaps in service provision.
