About Community Health Centers
This August, more than 14,000 health centers across the nation are celebrating over fifty years of serving communities. A community health center (CHC) is a nonprofit health care organization that provides high-quality, affordable, and comprehensive medical, dental, mental/behavioral health care (and more). Unlike other primary care systems, CHCs have a unique mission of providing access to underserved, underinsured, and uninsured patients. CHCs provide services to everyone, regardless of their ability to pay. They often offer discount programs to eligible patients based on their income and family size.

“Health Centers are determined to understand what our patients’ barriers to care may be, and we collaborate with the family to limit and mitigate these. We understand the importance of success in school as it relates to healthy outcomes, promote this idea, and support the family as the child grows.” – Darvi Rahaman, MD

Kids Matter!
Pediatric care is an important part of the CHC program, providing care to 8.8 million children nationwide. They can provide services like medical care, nutrition information, vaccinations, dental care, reading/education, and mental/behavioral health care. When providers work with children and their parents at an early age, there is a greater chance of establishing a healthy lifestyle to improve their future.

“We have focused on continuity of care, where a child sees their primary care provider and team consistently. Knowing your patient’s medical condition well only leads to better clinical quality outcomes.” – Darvi Rahaman, MD

Special Programs for Kids & Families
As a CHC, Peak Vista offers programs and services beyond primary care to help keep your family healthy.

Pediatric Health Center at International Circle
At Peak Vista’s Pediatric Health Center at International Circle, we provide medical, dental, and behavioral health care to children of all ages. Our team of family practice providers and pediatricians are here to help. We have convenient hours that will allow parents and caregivers the flexibility to schedule appointments. When kids grow up, they can easily transition out of the health center and see any of our family practice providers.

Well-Child Waiting Areas
Finding a babysitter during the day can be hard. It can often stand in the way of visiting the doctor. Our Well-Child Waiting Areas allow parents to drop off their healthy children so they can attend their clinical appointments – for free! We have three Well-Child Waiting Area locations.

First Visitor Program
Peak Vista’s First Visitor program provides free, one-on-one support for parents and caregivers in the comfort of their own homes. The program pairs families with children ages zero to five with a Family Support Lead Care Coordinator who:

  • Provides emotional support
  • Connects parents and caregivers with community resources
  • And shares information on health, nutrition, safety, and important developmental milestones with the family.

Children Youth and Family Services 0-3 Program
Peak Vista partners with the Department of Human Services (DHS) to provide care to families and children who may be experiencing abuse or neglect. A provider from Peak Vista joins the DHS caseworker on their house calls and completes a medical checkup on the entire family. They then work with the family to schedule appointments with providers, call in medication prescriptions, and connect to other community resources.

To make an appointment or to learn more about other Peak Vista services, visit peakvista.org or call (719) 632-5700.
